Great job! Next time I draw, I'll use Photoshop CS5. Nah, just joking. I probably couldn't even draw a smiley face on a computer.
It's all about translating what's in your mind onto the paper or screen in this case and having the patience and the obsession to do it A LOT. Also attention to detail helps as well.
I'd ask how you do it, but you just showed us, and I still don't understand how you can do all of that x)

Oh dear gosh I said "diseased" didn't I? I meant deceased...yeah she's pushing up daisies now...though she was probably diseased so that would explain a few if her many problems.

There's nothing to teach, just use the lasso tool and go for it XD

I can never get it to look right if it doesn't look perfect when I draw it the first time- which isn't easy considering photoshop lacks a useful pen tool and my mouse can be jittery *heavy sigh*

I survive on the transform tools XD too lazy to redraw anything especially if you can click and drag a few times and it's all good.
Wow, it's really cool seeing how you do it. We certainly go at it differently, I never touch the transform tools unless I'm working on a background.
